People experiencing homelessness often have complex health needs, with higher incidences of co-occurrent conditions than the general population. They are also admitted to hospital four times as often and stay three times as long.
Through models of care like its Partnership Programme, Pathway looks to maximise the potential of a hospital admission to ‘treat the person’ not just the condition, and achieve lasting positive health, social care, and housing outcomes. We also produce research and standards to further knowledge and practice in the field.
