This Pathway Team went live at the beginning of June 2022 covering Ealing and Northwick Park Hospitals. It is the sister team for Inner Northwest London. The team consists of two doctors, two nurses, an OT Nina service lead, and two housing link workers.

The team works with health, housing and social services, and the wider homelessness sector, to ensure that hospital inpatients get the best possible health impact from admission and are linked into appropriate housing and health services on discharge. They liaise with clinicians within the hospital to maximise referrals and increase knowledge and confidence in dealing with some of the complex needs of patients experiencing homelessness. They also advocate with housing departments to try and ensure that clients’ specific circumstances are considered, and accommodation is provided where possible.  In the event that patients are not eligible for statutory housing, they reach out to other charitable or commercial organisations to try and find satisfactory solutions. 

One of the team doctors has spoken about their work at a meeting of the All- Party Parliamentary Group for Ending Homelessness.
