Our first Pathway Partnership Programme annual report demonstrates how we work with NHS organisations to help them to improve care and outcomes for people experiencing homelessness.
Of key interest to NHS commissioners and mangers, the report draws on the data and collective experience of our Pathway Teams around the country. It highlights some of the extreme challenges facing homeless patients, and the parallel challenges we have faced when working in a new way, in collaborative, reciprocal partnership with large NHS institutions and ever-changing commissioning structures.
