Considering the impact housing and deprivation can have on the health of our local population, we piloted the Pathway model locally in East Kent for six months in 2022 with positive outcomes.
Through this partnership approach and following the successful pilot phase, the East Kent Homeless Pathway Team was subsequently established in September 2023 with funding from the East Kent Health and Care Partnerships Health Inequalities Fund. The team runs four GP sessions per week, and includes a full-time nurse, a support worker from the homelessness and mental health charity Porchlight, a local authority housing officer from Canterbury Housing Options, and an administrator.
The success of this partnership model resulted in the service receiving a Highly Commended award at the Kent Housing Group Excellence Awards 2023.
