A new Pathway team has gone live today in North West London.
The new inclusion health team has joined forces with Pathway in collaboration with Imperial NHS Trust, Chelsea and Westminster Hospital, St Mungo’s, Westminster Council, Central London Community Healthcare, Great Chapel Street Medical Centre, North East London CCG and North West London CCG.
This team has been fully co-designed, funded and developed in complete partnership. The inclusion health team will work and learn together to improve the quality of care delivered to vulnerable patients. As well as treating health care needs, the new team will help to resolve housing and social care issues.
Alex Bax, Chief Executive of Pathway, said:
“We are pleased to have helped set up this service in partnership with some inspiring colleagues. We know that patients who are homeless are more than twice as likely to be readmitted to hospital in an emergency compared to patients with housing. Having a dedicated team who are committed to caring for some of London’s most vulnerable people will help reduce this cycle of re-admittance to hospital and help people on to a more stable and secure pathway.”
Imperial NHS Trust saw their first inclusion health patients today. Chelsea and Westminster Hospital will start taking patient referrals through the inclusion health team from December 6th.
