This award is a tribute to all the fantastic Pathway Partnership teams working across London to ensure that those experiencing homelessness get the treatment and support they need.
Pathway aims to build on this success and continue to support our Pathway Partnership teams by investing our winnings in specialist reflective practice for our teams. As always, we will continue to develop our understanding of the best models of care for those experiencing homelessness and learning how best to share what works right across the system.
As part of the awards, a short-film was produced providing an insight into the inspiring work of the Homelessness Inclusion Team at St George’s Hospital.
The Homelessness Inclusion Team at St George’s Hospital is one of five Pathway Partnership teams based in hospitals across London, including St Georges, Croydon, St Mary’s/Imperial, Homerton, and Ealing and Northwick Park, that have signed up to the Pathway Partnership support programme and introduced Pathway’s model of care for patients experiencing homelessness. In return for an annual fee, each team receives support and training from Pathway’s core staff, as well as access to our online service manual and specialist support networks.
