The ongoing homelessness crisis is driving growing numbers of people facing homelessness into hospital, with Pathway’s specialist homelessness hospital teams supporting 28% more people than last year. 

Despite these growing pressures, Pathway’s multi-disciplinary teams working in hospitals to support patients facing homelessness have improved outcomes for 3,377 such patients, reducing their rough sleeping by 40% on average.  

The network of eight teams across England made an estimated cost saving of £4.8 million to the NHS.  

The Pathway Partnership Programme teams continue to deliver outstanding results, whichever lens is used to examine them. They support high numbers of people, make significant changes to their lives, and save the health system significant sums of money.
