From April 2023 to June 2024 Pathway led a Burdett Trust for Nursing funded collaborative project which examined ways to improve the nursing and allied health care of people experiencing homelessness with diabetes across the UK.
One output of the project was this free e-learning course designed to help practitioners improve outcomes for this vulnerable group, who typically experience poor health and challenges in accessing care.
Hosted by inclusion health education charity Fairhealth, the course looks at the initial challenges in providing care, including psychological, practical and condition-managements issues for patients, the system and service delivery issues that they might face, and issues of frequent attendance and self-discharge.
Learning for better practice includes clinical practice considerations for improving care, advice around nutrition, eye and foot health, guidance on safeguarding in cases of self-neglect, and advice on undertaking Quality Improvement projects in this area.
