This is the latest six-month evaluation report for the St George’s Homelessness Inclusion Team.
Their compassionate, person-centred approach has resulted in significant achievements in this period:
- 60% reduction in rough sleeping comparing hospital admission to discharge).
- Completion of Duty to Refer (statutory homelessness referral to a Local Authority) for 93% of eligible patients.
- Reduction in self-discharge rates of patients seen by the team from 14.4% reported in the previous evaluation to just 6.9% – thereby reducing readmission and reattendance.
- 60% of patients without a GP were assisted to register with a GP they could then access following discharge from hospital.
- 100% of service users surveyed gave feedback that the team was kind and caring at all times, made them feel completely safe and had been extremely helpful.
